Need a copy of your medical records? We’re here to help make the process simple.
To get started, please complete the Authorization for the Release of Medical Records form and include a copy of your photo ID. You can submit these in one of two ways:

• Drop off the form and ID with our receptionist
• Fax them directly to the Medical Records Department at 225-231-3026

Please allow up to 15 business days for processing.
